* Regex fix, checkip argument & updated tld list
This patch include 3 fixes:
1) The plugin could match invalid URIs, eg. "www..example.com", which resulted in fatal plugin error, sending the incoming email directly into queue without any further scan. The regex has been fixed.
2) Some URIBLs trigger positive on any ip-address. Added new plugin argument: checkip [yes|no].
3) Updating the TLD list.

1. the known users of async don't upgrade
2. async becomes a win when concurrent connections exceed a few hundred simultaneous
3. anyone that needs async should be looking at Haraka instead
4. the perl async dependencies aren't maintained
on files in plugins dir:
fixed a number of POD errors
formatted some # comments into POD
removed bare 1; (these are plugins, not perl modules)
most instances of this were copy/pasted from a previous plugin that had it
removed instances of # vim ts=N ...
they weren't consistent, many didn't match .perltidyrc
on modules that failed perl -c tests, added 'use Qpsmtpd::Constants;'

Jost Krieger pointed out that the documentation for the header check called
for a config_headers, but the code actually implemented scan_headers. Updated
to accept either. Also the condition for actually checking/skipping the
headers was inverted.
Also whitespace fixes.
